Background
Early screening and management of metabolic dysfunction-associated steatotic liver disease (MASLD) are essential to prevent progression to fibrosis, cirrhosis, and related chronic diseases. The neck circumference to height ratio (NHtR) is a reliable measure of upper body fat. This study explored the relationship between the NHtR and MASLD in the Korean population, with the aim of validating it as a reliable screening tool.
Methods
We analyzed data from the 2019 to 2021 Korea National Health and Nutrition Examination Survey, including 11,414 participants aged 40 years or older. The association between the NHtR and MASLD was evaluated using logistic regression analysis, while predictive accuracy was evaluated using receiver operating characteristic curve analysis.
Results
A total of 11,414 participants aged 40 years or older were included in the study. Participants in the highest NHtR quartile had higher levels of metabolic risk markers, including body mass index, waist circumference, glucose, hemoglobin A1c, and homeostatic model assessment of insulin resistance (P<0.001). In logistic regression analysis, the odds ratio for MASLD in quartile 4 compared to quartile 1 was 1.77 (95% confidence interval [CI], 1.31–2.40; P<0.001) in the fully adjusted model. The area under the curve and 95% CI of NHtR for MASLD were 0.749 (95% CI, 0.740–0.758), demonstrating superior accuracy compared with other indicators. The optimal NHtR cut-off for MASLD was 21.564, with a sensitivity and specificity of 0.726 and 0.644, respectively.
Conclusion
NHtR is a more effective predictor of MASLD than other measures, highlighting its potential as a valuable screening tool in clinical and self-care settings.

Background
Sickness presenteeism (SP) refers to the phenomenon where employees attend work while sick, a decision that impacts both their physical and mental well-being and can lead to burnout. This study aimed to determine the prevalence of and factors influencing SP among Thai and foreign teachers following the end of the coronavirus disease 2019 public health emergency of international concern (PHEIC).
Methods
A sample of 400 individuals was analyzed using chi-square tests and logistic regression to obtain odds ratios (ORs) and 95% confidence intervals (CI).
Results
Post-PHEIC, a significant proportion of teachers (51.9%) perceived an increase in their workload, and 62.1% reported having taken health-related leave of absence. The prevalence of SP was high (35.0%). Factors significantly associated with SP (P<0.05) included being younger than 37 years (OR, 1.825; 95% CI, 1.108–3.006), facing monthly financial challenges (OR, 1.627; 95% CI, 1.028–2.575), doing school-related work on holidays (OR, 1.920; 95% CI, 1.180–3.122), and experiencing sleep disturbances owing to work-related stress (OR, 1.910; 95% CI, 1.203– 3.033). No discernible difference existed in SP rates between Thai and foreign teachers.
Conclusion
As a course of action, we recommend the provision of appropriate financial planning resources for teachers. Policywise, school events and activities should be scheduled on working days to ensure that teachers maintain a work-life balance. Additionally, task allocation that caters to individual strengths and limitations can mitigate the effects of SP.

Efficiency of Associating Therapeutic Patient Education with Rehabilitation in the Management of Chronic Low Back Pain: A Randomized Controlled Trial

Background
This study aimed to assess the benefits of associating rehabilitation with therapeutic patient education (TPE) to decrease fear-avoidance belief and pain and improve function in adults with chronic low back pain (CLBP).
Methods
This randomized controlled study included 100 patients with CLBP according to the CONSORT (Consolidated Standards of Reporting Trials) guidelines. The patients were divided into two teams: group A that participated in the TPE in association with rehabilitation and group B that received rehabilitation only. Pain and functional amelioration were assessed initially (T0) and at the end of the program (T1) using a visual analog scale at rest, work, and activity, and the Echelle d’Incapacité Fonctionnelle pour l’Évaluation des Lombalgies scale. Psychological and apprehension and avoidance assessments were also conducted, including the evaluation of depression, anxiety, fear-avoidance belief, and kinesiophobia using the Hospital Anxiety and Depression Scale, Fear-Avoidance Beliefs Questionnaire, and Tampa scale of kinesiophobia scale.
Results
The evaluation of progression initially (T0) and then at the end of the program (T1) revealed a significant reduction in pain at rest (P=0.00) and while working (P=0.00) and doing physical activity (P=0.03); a decrease in anxiety (P=0.03), fear-avoidance belief (P=0.03), and kinesiophobia (P=0.02); and an improvement in function (P=0.00) for patients in group A without amelioration of depression (P=0.15). Concerning group B, we identified a significant regression in pain at rest (P=0.001) and while working (P=0.03) and doing physical activity (P=0.00); depression (P=0.01); fear-avoidance beliefs (P=0.00); and kinesiophobia (P=0.002). Comparison between the groups revealed that associating TPE with rehabilitation resulted in a more significant improvement in function (P=0.00), anxiety (P=0.00), fear-avoidance belief (P=0.00), and kinesiophobia (P=0.00).
Conclusion
Associating TPE with rehabilitation improved function and reduced fear, false beliefs, and kinesiophobia of movement in patients with CLBP.

Relationship between Serum Testosterone Level and Carotid Intima-Media Thickness among Korean Men and Postmenopausal Women

Background
Given that the role of serum testosterone on incident cardiovascular disease has been uncertain, it is necessary to find out the relationship between serum testosterone and carotid atherosclerosis.
Methods
The study participants included 1,302 Korean adults (873 men and 429 postmenopausal women) who participated in the Healthy Twin Study and were not receiving androgen deprivation therapy. The participants were classified into three groups: men aged <40 and ≥40 years and postmenopausal women. Total testosterone (TT) and sex hormone-binding globulin (SHBG) concentrations were measured using electrochemiluminescence immunoassays, and free testosterone (cFT) levels were calculated using Vermeulen’s method. Carotid intima-media thickness (IMT) was measured at three levels using a high-resolution B-mode ultrasound equipped with a 7-MHz linear transducer. The associations between sex hormone concentrations and carotid IMT were evaluated using a mixed linear regression analysis.
Results
After adjusting for cardiovascular risk factors, TT was found to be inversely associated with common carotid IMT in men aged ≥40 years, with a 4.5% decrease in common carotid IMT for every one-standard deviation increase in TT concentration (P=0.0063). In contrast, TT was not significantly associated with carotid IMT in all segments in men aged <40 years and postmenopausal women. Additionally, SHBG and cFT were not associated with carotid IMT in any segment.
Conclusion
The significant association between TT level and common carotid IMT in men aged ≥40 years suggests that decreased testosterone levels are involved in the development of atherosclerosis in men.

The Correlation between Carotid Intima-Media Thickness and Neutrophil to Lymphocyte Ratio in Prediabetes Patients

Background
Prediabetes is a metabolic state between normoglycemia and diabetes and is known to carry a higher risk of developing overt diabetes and cardiovascular disease (CVD). The relative and absolute risks of all-cause mortality, CVD, coronary heart disease, and stroke in prediabetes patients, as well as in diabetic patients, is higher than that in patients with normoglycemia. Carotid intima-media thickness (cIMT) is a method used to stratify CVD risk. In this study, we aimed to determine whether the neutrophil-to-lymphocyte ratio (NLR) correlates with cIMT in prediabetes patients.
Methods
From January 1, 2016, to February 20, 2021, 581 adults their 30s–70s who underwent carotid ultrasonography as part of a comprehensive medical examination at the Dongtan Sacred Heart Hospital were enrolled. Statistical analysis using SPSS presented t-test and chi-square test significance levels into a group with normal cIMT (nIMT; cIMT <1 mm) and a group with thick cIMT (tIMT; cIMT ≥1 mm). Binary logistic regression analysis was performed to confirm the correlation between NLR and cIMT.
Results
In prediabetic adults, age, hemoglobin A1c (HbA1c), systolic blood pressure, and NLR were significantly higher in the tIMT group than in the nIMT group. In the regression analysis, NLR, age, and HbA1c were significantly correlated with cIMT.
Conclusion
NLR was significantly higher in the tIMT group than in the nIMT group; therefore, NLR may be used to assess CVD risk in prediabetes patients.

Comparison of Physiotherapy Approaches in Low Back Pain: A Randomized Controlled Trial

Background
The purpose of this study was to compare the effectiveness of rehabilitation approaches in individuals with low back pain (LBP) on pain, spinal mobility, disability, and muscular strength.
Methods
Ninety volunteers were included and divided into four groups depending on the rehabilitation approach: group 1, soft tissue mobilization techniques and stabilization exercises (n=24; 11 females [F], 13 males [M]); group 2, Kinesio Taping and stabilization exercises (n=24; 12F, 12M); group 3, stabilization exercises (n=22; 11F, 11M); and group 4, reflex therapy and stabilization exercises (n=20; 10F, 10M). Visual Analog Scale for pain intensity, an isokinetic evaluation for strength at 60°/s and a side-plank position test for trunk stabilization were measured before and assessed at the beginning, after a 4-week treatment and during 4 weeks of follow-up. The functional status was evaluated with the Oswestry Disability Index.
Results
Individuals in all groups showed similar decrease in pain after the treatment and at 1-month follow-up, but there were no significant differences in pain levels between the groups (P<0.05).
Conclusion
All therapeutic approaches were found to be effective in diminishing pain and thus helpful in increasing strength and stabilization in patients with LBP.

Sickness Certification Behavior amongst Primary Care Physicians: A Descriptive Analysis of Indirect Measures Using the Theory of Planned Behavior

Background
The provision of sick leave to patients in general practice often entails a complex decision-making process. Commonly, general practitioners believe that the act of providing sick leave can become confrontational. We assessed the intention, attitude, subjective norm, and perceived behavioral control of general practitioners in relation to sick leave provision at public health clinics in Malaysia.
Methods
A cross-sectional study design was implemented using data from 86 primary health care clinics in two states in Malaysia from February 2014 to March 2015. A questionnaire that comprised indirect measures were developed on the basis of the Theory of Planned Behavior. The questionnaire assessed several dimensions related to sickness certification provision viz., intention, attitude, subjective norm, and perceived behavioral control.
Results
The mean scores in this study revealed that physicians acknowledged that patient factors such as clinical symptoms (6.59±0.04), debilitating signs and symptoms of diseases (6.45±0.06), importance of illness recovery (6.07±0.07), and approval of employers (5.35±0.09) played an important role in their decision to provide sick leave.
Conclusion
The act of providing sickness certification to patients is a complex decision-making process. The findings of this study can be used to devise a targeted intervention to reduce the conflict that physicians face in issuing sick leaves.

Chronic Back Pain in a Young Female Patient: A Case of Ependymoma Originating from the Conus Medullaris

We present the case of a 14-year-old Malay girl with an ependymoma of the conus medullaris who presented to multiple general practitioner clinics with a 24-month history of chronic low back pain. The pain was symptomatically managed as a simple musculoskeletal pain and sciatica. Further imaging to aid diagnosis was delayed until the appearance of severe pain with neurological deficits. Magnetic resonance imaging revealed an enhancing spinal mass at L1 through L3, and histopathological investigations confirmed the grade II ependymoma according to the World Health Organization classification. She underwent gross resection of the tumor. After the surgery, she developed neurogenic urinary bladder and bowel, which required intermittent self-catheterization, intermittent enema use, and intensive physical therapy.

BCL11A rs1427407 Genotypes in Sickle Cell Anemia Patients Undergo to Stroke Problems in Sudan

Background
Sickle cell disease is an autosomal recessive condition that results from the presence of a mutated form of hemoglobin. Some genetic variants of BCL11A are amenable to therapeutic manipulation. The present study investigated the relationship of a BCL11A variant (rs1427407) and its plasma levels with vaso-occlusive crises and stroke complications among patients in Sudan with sickle cell disease.
Methods
This cross-sectional study was performed between June 2014 and October 2016. The subjects included 166 patients who were diagnosed with sickle cell disease and 35 healthy control subjects, who were grouped according to sex and age (<15 years, 15–25 years, and >25 years). All patients and/or their guardians provided informed consent. Blood samples were collected from the patients and controls under aseptic conditions.
Results
Plasma BCL11A levels were elevated in cases with vaso-occlusive crises that lasted for >3 years. In addition, plasma BCL11A levels were high in cases with the GG genotype (vs. GT and TT) at rs1427407. Furthermore, the BCL11A rs1427407 GG/GT genotypes increased the risk of vaso-occlusive crisis and stroke in the patients with sickle cell disease.
Conclusion
The BCL11A variant (rs1427407) and its plasma levels were associated with vaso-occlusive crisis and stroke in patients with sickle cell disease.

Comparison of Patient-Physician Interaction Scores of Clinical Practice Examination between Checklists and Rating Scale

Background

The clinical practice examination (CPX) was introduced in 2010, and the Seoul-Gyeonggi CPX Consortium developed the patient-physician interaction (PPI) assessment tool in 2004. Both institutions use rating scales on classified sections of PPI but differ in their scoring of key components. This study investigated the accuracy of standardized patient scores across rating scales by comparing checklist methods and verified the concurrent validity of two comparable PPI rating tools.

Methods

An educational CPX module dyspepsia case was administered to 116 fourth-year medical students at Hanyang University College of Medicine. One experienced standardized patient rated exams using two different PPI scales. She scored checklists composed of 43 items related to the two original PPI scales through video clips of the same students. From these checklists, we calculated Pearson's correlation coefficient.

Results

The correlations of total PPI score between the checklist and rating scale methods were 0.29 for the Korean Medical Licensing Examination (KMLE) tool and 0.30 for the consortium tool. The correlations between the KMLE and consortium tools were 0.74 for checklists and 0.83 for rating scales. In terms of section scores, the consortium tool showed only three significant correlations between the two methods out of seven sections and the KMLE tool showed only two statistically significant correlations out of five sections.

Conclusion

The rating scale and checklist methods exhibited a weak relationship in the PPI assessment, but a high correlation between assessment tools using the same method. However, the current rating scale requires modification by reorganizing key scoring components through factor analysis.

Background

The main aim of this study was to evaluate the achievements of some important goals of Iran's urban family physician plan. This plan was implemented when the country experienced economic instability. We examine whether an economic crisis affects the efficacy of a healthcare program.

Methods

We used the household income and expenditures survey data for 2011 (before program implementation) and 2012 (after program implementation). Changes in out-of-pocket payments and healthcare utilization were investigated using the propensity score matching estimator. Furthermore, changes in inequality in these two dimensions were examined.

Results

No changes in out-of-pocket payments and healthcare utilization were found after the implementation of this program; however, inequality in out-of-pocket payments increased during the reform.

Conclusion

The urban family physician program was not implemented completely and many of its fundamental settings were not conducted because of lack of necessary healthcare infrastructure and budget limitations. Family physician programs should be implemented under a strong healthcare infrastructure and favorable economic conditions.

Association of 10-Year Atherosclerotic Cardiovascular Disease Risk Score with Carotid Intima-Media Thickness and Plaque

Background

The aim of this study was to determine the association between carotid intima-media thickness (IMT) and newly developed 10-year atherosclerotic cardiovascular disease (ASCVD) risk score.

Methods

Participants were 201 Korean adults who underwent carotid ultrasonography at the Health Promotion Center of the Eulji General Hospital. We obtained information about medical history and lifestyle, and conducted laboratory tests. Carotid IMT by B-mode ultrasonography was measured. Carotid injury was considered present when the maximum carotid IMT was ≥0.9 mm or when arteriosclerotic plaques were detected. The 10-year ASCVD risk score was calculated using the 2013 American College of Cardiology/American Heart Association guidelines.

Results

Men had higher 10-year ASCVD risk score than did women (mean±standard deviation: 7.15±6.04 vs. 2.53±3.67, respectively; P<0.001). Ten-year ASCVD risk was significantly correlated with right maximum carotid IMT (r=0.307), left maximum carotid IMT (r=0.230), right mean carotid IMT (r=0.322), and left mean carotid IMT (r=0.264). The group with high 10-year ASCVD risk were at even higher risk of carotid injury than were the group with low 10-year ASCVD risk (odds ratio, 2.201; 95% confidence interval, 1.162-4.1706; P=0.019). Only 10-year ASCVD risk score was significantly associated with carotid injury (odds ratio, 4.104; 95% confidence interval, 1.570-10.729). Variables that were not included in the 10-year ASCVD risk score were not significantly associated with carotid injury.

Conclusion

The findings of this study suggest that 10-year ASCVD risk score is associated with carotid injury.

Relationship between Neck Length, Sleep, and Cardiovascular Risk Factors

Background

Neck circumference, as a predicator of obesity, is a well-known risk factor for obstructive sleep apnea and cardiovascular diseases. However, little research exists on neck length associated with these factors. This study explored the association of neck length with sleep and cardiovascular risk factors by measuring midline neck length (MNL) and lateral neck length (LNL).

Methods

We examined 240 patients aged 30 to 75 years who visited a health check-up center between January 2012 and July 2012. Patients with depressive disorder or sleep disturbance were excluded from this study. MNL from the upper margin of the hyoid bone to the jugular notch and LNL from the mandibular angle to the mid-portion of the ipsilateral clavicle were measured twice and were adjusted by height to determine their relationship with sleep and cardiovascular disease risk factors.

Results

Habitual snorers had shorter LNL height ratios (P = 0.011), MNL height ratios in men (P = 0.062), and MNL height ratios in women (P = 0.052). Those snoring bad enough to annoy others had shorter MNL height ratios in men (P = 0.083) and women (P = 0.035). Men with objective sleep apnea had longer distances from the mandible to the hyoid bone to the mandible (P = 0.057). Men with metabolic syndrome had significantly shorter LNL height ratios (P = 0.021), and women with diabetes, hyperlipidemia, and metabolic syndrome had shorter MNL height ratios (P < 0.05).

Conclusion

This study shows that a short neck by measuring the MNL is probably associated with snoring. In addition, MNL is related to cardiovascular disease risk factors in women.

Gallstones are Associated with Intima-Media Thickness of Common Carotid Arteries in Men

Background

The prevalence of gallstone disease is increasing in Korea. Gallstone disease and atherosclerosis share common risk factors. Ultrasonographic carotid intima-media thickness is an independent predictor of atherosclerosis. We measured the carotid intima-media thickness among men who had gallstones to establish the association between atherosclerosis and gallstones.

Methods

A total of 330 men who visited the health promotion center of the IS Hallym Hospital from January 2011 to July 2012 were recruited. Both abdomen and carotid ultrasonographies were performed. Demographic, anthropometric, and biochemical data were collected.

Results

The prevalence of gallstones was 6.6%. The carotid intima-media thickness, age, and body mass index were higher in subjects with gallstones (P < 0.05 for all). After Pearson correlation analysis, gallstone, age, body mass index, and fasting blood sugar were significantly associated with carotid intima-media thickness. A statistically positive association between gallstone and carotid intima-media thickness was demonstrated in a multivariate analysis adjusted for age, body mass index, and fasting blood sugar.

Conclusion

Carotid intima-media thickness is higher in male subjects with gallstones, indicating that they are at greater risk for atherosclerosis.

Thoracolumbar Junction Syndrome Causing Pain around Posterior Iliac Crest: A Case Report

Thoracolumbar junction syndrome is characterized by referred pain which may originate at the thoracolumbar junction, which extends from 12th thoracic vertebra to 2nd lumbar vertebra, due to functional abnormalities. Clinical manifestations include back pain, pseudo-visceral pain and pseudo-pain on the posterior iliac crest, as well as irritable bowel symptoms. During clinical examination, pain can be demonstrated by applying pressure on the facet joints or to the sides of the spinous processes. Radiological studies show only mild and insignificant degenerative changes in most cases. We report a 42-year-old female patient with osteogenesis imperfecta who suffered from chronic low back pain. Under the diagnosis of thoracolumbar junction syndrome, she was treated with an epidural block and a sympathetic nerve block, which improved her symptoms.

Factors Related to Pre-Procedural Anxiety for Gastroscopy in Health Check Examinee.

Background
Gastroscopy is the useful method to detect the upper gastrointestinal condition, but examinee have recognized it as the procedure causing the discomfort and anxiety. This study was conducted to identify the factors related to pre-procedural anxiety for gastroscopy .Methods: From 9th March to 12th June 2009, Data were collected from 463 examinee undergone gastroscopy at a health promotion center of a university hospital by self-administered questionnaires about general characteristics (age, sex, education state, economic state, family history of gastric cancer), gastrointestinal symptom, histroy of gastrointestinal disease, sedated gastroscopy and previous experience of gastroscopy. Before gastroscopy procedure, pre-procedural anxiety was assessed using 20-items by Korean version of Spielberger State Triat Anxiety Inventory Form-Y (STAI-Y).Results: The mean score of pre-procedural state anxiety was 38.1 ± 10.2 in all participants. The mean score was the most highest in participants who had no previous experience of gastroscopy and would perform non-sedated gastroscopy as 42.1 ± 10.3. Factors related to pre-procedural anxiety were female (odds ratio [OR], 3.00; 95% confidence interval [CI], 1.94 to 4.64), low education state (OR, 1.68; 95% CI, 1.05 to 2.71) and the previous experience of gastroscopy (OR, 0.46; 95% CI, 0.24 to 0.88). Among participants who had the previous experienced gastroscopy, female (OR, 3.20; 95% CI, 1.97 to 5.22), low education state (OR, 1.79; 95% CI, 1.06 to 3.02) and the good tolerance at previous gastroscopy (OR, 0.35; 95% CI 0.21 to 0.59) were related to pre-procedural anxiety. Conclusion: In health check examinee, female, low education state and the no previous experience of gastroscopy were related to pre-procedural anxiety. Endoscopists should be provide efforts to lower pre-procedural anxiety in those subjects as to increase effectiveness of interventions.

Background
Angiotensin converting enzyme inhibitors (ACEIs) or Angiotensin II type 1 receptor blockers (ARBs) are compelling indication drugs for hypertensive patients with diabetes mellitus. But prescription rate in 2005 year study of Pohang · Gyeongju area was only 30.8%. Therefore, a study on the change of prescription rate in the same area after 3 years was done.Methods: During three months from January 2008, 152 hypertensive patients with diabetes mellitus on their prescribed antihypertensive medications by 9 family physicians in visiting order were analyzed. After the analysis, the infl uencing factors for such prescriptions were ascertained by directly visiting each physicians who prescribed them. Results: A regimen of 16 antihypertensive agents were prescribed by these family physicians. Prescription count of ACEIs or ARBs was 101 cases (66.4%). ACEIs single therapy was 19 cases (12.5%), ACEIs combination therapy was 7 cases (4.6%), ARBs single therapy was 31 cases (20.4%) and ARBs combination therapy was 44 cases (28.9%). The ACEIs or ARBs which were selected by physicians that followed "compelling indication" was 5 (55.6%), "excellent reduce pressure effect" was 3 (33.3%) and "public relations of new medicine" was 1 (11.1%).Conclusion: In prescribing antihypertensive agents for patients with diabetes mellitus, selection of ACEIs or ARBs was increased from 30.8% to 66.4%. Education of recommended standard by participating in such study and developing of excellent new medicines may increase such change.

Association between Smoking Status and Insulin Resistance in Apparently Healthy Korean Men.

Background
Several studies have suggested that smoking may cause insulin resistance. However, the association between smoking and insulin resistance is still controversial. The purpose of this study was to investigate the association between smoking status and insulin resistance in Korean nondiabetic male population.Methods: A total of 5,969 men, aged > 20 years were recruited from those who visited the Health Promotion Center, Samsung Medical Center between 2005 and 2006. All subjects were divided into three categories: on-smokers (n = 2,594), ex-smokers (n = 1,580), and current-smokers (n = 1,795). Fasting values for glucose and insulin were used to estimate insulin resistance by HOMA (homeostasis model assessment). An independent association between smoking status and HOMA-IR (homeostasis model assessment of insulin resistance) was assessed after adjustment for factors infl uencing insulin sensitivity such as age, exercise, alcohol, body mass index, abdominal circumference, and blood pressure.Results: HOMA-IR was signifi cantly higher in ex-smokers and current-smokers than in non-smokers (2.09 ± 0.94 vs. 2.04 ± 0.90, 1.96 ± 0.86, P < 0.01, respectively). After adjustment for factors potentially affecting insulin sensitivity (age, alcohol intake, exercise, BMI, body fat, abdominal circumference, and blood pressure), the association persisted for ex-smokers (P = 0.008). And HOMA-IR was signifi cantly higher in ≥ 40 pack-years smokers than in non-smokersConclusion: Based on HOMA-IR, previous-smoking and chronic smoking were significantly associated with insulin resistance in apparently healthy Korean nondiabetic men.

Prevalence and Associated Factors of Neck Shoulder Pain in High School Students.

Background
Neck shoulder pain (NSP) was highly prevalent even among the adolescents in previous Western studies. However, the prevalence among Korean adolescents was seldom studied and the factors associated with NSP were controversial. Methods: A self-administered questionnaire asking the presence of NSP, restriction in daily activities due to NSP, behavioral characteristics, and psychological factors was given to 1,232 adolescent enrolled in high schools. Finally, 1,194 providing adequate responses to the questionnaire were included. NSP occurring once a week or more over the last 6 months was defined as a significant NSP and the association between the significant NSP and the studied variables was estimated using multiple logistic regression analysis. Results: The prevalence of NSP and the significant NSP were 82.9% and 33.4%, respectively. The girls (40.6%) were experiencing the significant NSP more than the boys (26.0%). Restriction in daily activities due to NSP was very common and 74.8% of the girls and 65.2% of the boys reported study disturbance. The factors associated with higher risk significant NSP in the boys were higher BMI (25≥ kg/m2), moderate stress, and fairly poor or poor self-assessed physical condition with odds ratios (95% confidence intervals) of 1.86 (1.02∼3.40), 1.74 (1.03∼2.97), and 2.30 (1.26∼4.19), respectively. In the girls, a longer (>75% of total sitting time) time of sitting in desk/chair of the inadequate height was associated with a higher risk of the significant NSP (odds ratio: 1.59, 95% confidence interval: 1.05∼2.39). Conclusion: The prevalence of NSP in Korean adolescents was very high, suggesting that active efforts to prevent NSP in Korean adolescents would be needed with a consideration of different risk factors between girls and boys. The Evaluation of Foreign Graphic Health Warning Labels on Smoking Cessation Effectiveness.

Background
Many countries have been legally prescribing graphic health warning labels on cigarette package as a part of their national policy of smoking cessation. This study was designed to evaluate the effectiveness and appropriateness of graphic health warning labels of Canada, Singapore and European union, in Korean for smoking cessation. Methods: From February to July in 2005, we surveyed 110 people who were high school students and college students. After showing them the 64 graphic health warning labels (16 from Canada, 6 from Singapore and 42 from European union), the self recorded questionnaires were collected. Results: The effectiveness for smoking cessation was related to the arousal levels of visual effects and it had the same result for each country in which we researched. The high arousal loss-framed graphic health warning labels were more effective than the gain-framed low arousal ones. Conclusion: To quit smoking, it can be reasonably concluded that high and negative images of health that were shown on warning labels of cigarette packages were effective. Hereafter, it is recommended more useful and proper designs of graphic warning labels be developed and applied. (J Korean Acad Fam Med 2007;28:923-930)

Factors Affecting Resident Physician's Underreporting Behavior of Needle Stick Injuries in a Tertiary Teaching Hospital.

Background
The most frequent hospital- related work injury is needle stick injury (NSI) that is likely to be experienced by most of the trainees during their residency, but not reported. Physicians under residency training were surveyed to identify the factors associated with non- reporting behavior of NSI according to health belief model. Methods: We surveyed 463 residents in a tertiary care teaching hospital in South Korea. A questionnaire on experiencing NSI, reporting behavior and the reasons for reporting behavior as done and collected between June and July 2004 via hospital groupware, AMCnet. The questionnaire was based on health belief model. Results: The e-mail questionnaire survey was received by 322 resident physicians and was completed by 43.8% (141). Among the responders, 88.7% (125) answered to have experienced needle stick injuries, and only 40% (50) had ever reported the incidence of NSI. The years of training, the infectivity of the patient in whom the needle was used, reporting mechanism or requirement and efficacy expectation were statistically significant factors for non-reporting behavior. Conclusion: The majority of the resident physicians did not report their NSI, and the variables affecting their underreporting behavior were evaluated by the health belief model. From this study, we learned that as the susceptibility of infection, the knowledge of reporting requirement and mechanism, and their expectation for the efficacies of reporting were higher, the reporting rate of their needle stick injury was likely to be higher as well. : It is reported that many types of psychiatric disorders, including anxiety disorder, are related with abnormal brain wave activity, and neurofeedback is associated with clinical improvement in generalized anxiety, OCD, phobic disorder, and PTSD. But in panic disorder, previous studies with neurofeedback are very lacking. Therefore, in this study, the author applied neurofeedback to patients with panic disorder, refractory to cognitive- behavioral therapy and medication.

Methods : From March 2 to May 15 in 2005, six patients with a panic disorder had received 20 sessions of neurofeedback training. The author evaluated the treatment effect by using the alteration of K-APPQ score, the frequency of panic attack, medication and individual handicap.

Results : All 6 patients completed 20 sessions of neurofeedback training. At the end, the score of total APPQ, agoraphobia, and interoceptive fear was decreased (P= 0.028) and the frequency of panic attack, medication, and individual handicap was also decreased.

Conclusion : Neurofeedback training was successful in reducing panic symptom severity, frequency and individual handicap. A controlled study on a larger population is strongly recommended.

Selection of Antihypertensive Drugs in Hypertensive Patients with Diabetes Mellitus: In Pohang-Gyeongju Primary Care Research Network.

Background
: Angiotensin converting enzyme inhibitor or Angiotensin II type 1 receptor blocker is usually recommended for hypertensive patients with diabetes mellitus. No reports have been made on antihypertensive agents prescribed for these patients in private practice.

Methods : During a one month period of December 2004, 169 hypertensive patients with diabetes mellitus on their prescribed antihypertensive agents by ten family physicians were analyzed. The influencing factors for such prescriptions were analyzed by directly visiting each physicians who prescribed them.

Results : A total of 21 antihypertensive agents were prescribed by these family physicians. Single calcium channel blocker therapy was the most common with 55 cases (32.5%), followed by 20 cases (11.8%) of single angiotensin converting enzyme inhibitor therapy and 16 cases of single therapy of Angiotensin II type 1 receptor blocker. The low frequency of prescribing the Angiotensin II type 1 receptor blocker was due to insufficient effect (7), lack of information (5), resistance from the patients from changing the medications (4) and expensive costs (4).

Conclusion : In prescribing antihypertensive agents for patients with diabetes mellitus, angiotensin converting enzyme inhibitor or Angiotensin II type 1 receptor blocker were chosen less. In order to decrease the incidence of complications in these patients, such agents should be prescribed more.

A Survey on the Effect of Cigarette Warning Labels.

Background
: In an effort to use cigarette warning labels as a means for reducing smoking rates, this study examined current cigarette warning messages and newly designed pictorial warning labels.

Methods : From June 15, 2005, to June 22, 2005 we conducted an e-mail survey of 1,200 civilians to examine the effect of cigarette warning labels. For statistical analysis, we used chi-square test with SPSS 11.0 software.

Results : Among all the respondents, 78.7% indicated that current cigarette warning labels did not have any effect. However, most of the respondents indicated that pictorial warning labels are effective in raising public awareness (84.6%) or knowledge (81.5%) of the harmful effects of cigarettes. They also responded that pictorial warning labels would help them quit smoking (75.9%) or keep them from buying cigarettes (64.1%). Over 80% of the respondents and over 70% of the smokers responded that they would welcome the use of pictorial warning labels as an anti-smoking measure.

Conclusion : The results of this study showed that current cigarette warning labels are not effective in curbing smoking rates. Also, the results suggest that current cigarette warning labels need to be improved if they are to be used as an effective anti-smoking measure.

The Family Physician's Role in Identifying and Treating Tobacco Addiction among Adolescents.

Smoking and tobacco addiction are serious public health problems worldwide. New research reveals that addiction to tobacco can begin very early, with very low levels of smoking. Family physicians are in a unique position to prevent smoking initiation by youths and to diagnose and treat tobacco addiction in young smokers. In this paper we discuss the factors that prompt youths to try smoking, how quickly addiction to tobacco begins after the onset of smoking, how a family physician can determine whether a young patient is addicted, and what the physician can do to prevent adolescent patients from beginning to smoke or to assist them to quit if they already smoke.

Predictors of High Peak Serum CK Levels in Doxylamine Intoxication.

Background
: Because doxylamine succinate (DS) is an over-the-counter medicine, it can be obtained easily and is frequently used in suicidal attempts. Patients usually recover without serious complications, but occasionally rhabdomyolysis and even death can occur in DS intoxication. In this study, the authors tried to find out the independent predictors of high peak serum CK levels, i.e. probable rhabdomyolysis in DS intoxication.

Methods : The medical records of 41 patients who visited a hospital for DS intoxication from January 1, 2002 to April 30, 2003, were reviewed retrospectively.

Results : In the group of DS only, initial occult blood of urine (P=0.003), initial WBC count (P=0.003) and confusion (P= 0.007) were the best predictors of the peak serum CK level (r2=0.724). In the group of DS with other drugs intoxication, initial creatinine level (P=0.003) and initial occult blood of urine (P=0.007) were the best predictors of the peak serum CK level (r2=0.784). In the cases of rhabdomyolysis patients, the time taken for the CK level to be increased over 1,000 IU/L was 1.9±0.6 days.

Conclusion : In DS only intoxication, occult blood in initial urine analysis, initial high WBC count and confusion can be thought of as useful clinical predictors for high peak serum CK level case. In DS with other drugs intoxication, initial creatinine level and initial occult blood of urine can be considered as the best predictors. More than 2 days will be needed for the observation of serious complications in DS intoxication.

Prediction of Adolescence Overweight from Childhood Body Mass Index 7 Year Retrospective Study of Suburban School Children.

Background
: Childhood and adolescent obesity, not only increases the risk of obesity into adulthood but also is a significant culprit affecting health in general. The purpose of this study was to examine the trend in body mass index (BMI) and to investigate the prediction of adolescence overweight from childhood weight status by retrospectively tracking for seven years.

Methods : The subjects, 520 adolescents aged 14 and 15 years, were recruited from a junior high school that is located in Guri-city. The subjects' height and weight measurements were extracted from the school records of annual physical examination. We assessed their BMI over a period of seven years starting at age seven. Other baseline data were obtained by questionnaires which were completed by both the subjects and their parents in 2001.

Results : The prevalence of overweight, according to the Korean Pediatric Society guideline, increased at age 7 through 14. The overweight adolescents of age 14 were more likely to have an obese mother. The results showed that 84% of males and 47% of females who were overweight in childhood continued to remain overweight in adolescence. Compared to the non-overweights, the relative risk of becoming an overweight adolescent among overweight boys was 5.7 (95% CI: 4.1∼8.1) and among overweight girls was 6.3 (95% CI: 3.4∼11.4).

Conclusion : Approximately 70% of the overweight children continued to remain overweight as adolescents. Therefore, prevention and effective management of obesity during childhood and adolescence are essential.

Caudal Epidural Steroid Injections for the Treatment of Chronic Lower Back Pain.

Background
: The purpose of this study was to evaluate the efficacy of caudal epidural steroid injection with local anesthetic for chronic lower back pain patients.

Methods : The 152 patients with lower back pain were selected for the study. The patients were treated with the caudal injections of steroid (triamcinolone acetate 40 mg) and local anesthetics (1% lidocaine 17 ml). The efficacy of caudal injections was assessed with the visual analogue scale (VAS) before procedure and at 8 weeks following procedure.

Results : The VAS scores of 'Before procedure' group and '8 weeks after procedure' group were 62±23 and 31±15, respectively. The caudal steroid injections provided effectiveness in treating various lower back pain diseases except postlaminectomy syndrome.

Conclusion : We consider that caudal epidural injections of steroid and local anesthetics are effective for patients with chronic lower back pain in the short term.

The Impact of the Change of Leisure Time Physical Activities on Body Weight.

Background
: The risk of overweight and obesity to health is well know. Physical activity is important in the treatment of obesity in terms of healthy body composition and maintenance of body function. This study is a follow-up study to identify the impact of changes of leisure time physical activities on body weight.

Methods : The study subjects were people who visited a health examination center in Seoul at least twice between 1995 and 2000. Seven hundred people who completed physical activity questionnaires each time they visited were included. Three hundred eighty two of them were men, and 318 were women. The questionnaire was Baecke physical activity questionnaire, consisted of items for work activities, sports activities, and non-sports leisure activities. Each index of activities was calculated.

Results : Ninety seven people (13.9%) reduced their weights more than 5% of initial body weights. Twenty eight people (4.0%) gained their weights more than 5% of initial body weights. In five hundred and seventy five people (82.1%) their body weights were changed within 5% of initial body weights. Non-sports leisure time indices increased greatly and were significantly more than sports indices in reduced weight group. Non-sports leisure time indices were not changed significantly in gained weight group. The dose-response relationship existed between changed body weight (kg) and difference of leisure time indices. The multivariate analysis showed that female was a significant factor related to reducing weight (odds ratio=4.5, 95% confidence interval 2.7~7.5). Increase of non-sports leisure indices showed borderline significance (odds ratio=1.4, 95% confidence interval 0.9~2.0).

Conclusion : To increase non-sports leisure activities for the purpose of reducing body weight is actively recommendable. Such activities performed on a daily basis may have impact on reducing weight through repeatability and ease of access.

The efficacy of Kinesio taping in patients with a low back pain.

Background
: Kinesio taping is a treatment for pain and dysfunction of musculoskeletal system, using tapes which have a similar elasticity to skin. This study was done to find out about th efficacy of kinesio taping on patients with a low back pain.

Methods : This study was performed in patients with a non-specific low back pain who had visited the Inchon International Airport Construction Authority Clinic from January 2000 to April 2000. We performed taping to a randomized case group and a placebo to control group during the first 3 days. After the first 3 days, we assessed the changes of improvement in low back pain with visual-analogue pain scale(VAS). From the second visit on, we also started carrying out kinesio taping in the control group.

Results : The total number of patients participating in this study was 43, but 4 patients did not complete the study stopped. After the first 3 days, control group showed just 0.93 of the VAS score improvement, while randomized case group showed 2.55 (p=0.003). The VAS score of case and control group were 3.18 and 3.03 respectively, which showed improvement at the end point of treatment compared with the first score.

Conclusion : Kinesio taping was more efficacious than placebo in patients with a nonspecific low back pain.

Clinical observation of patients with tsutsugamushi disease at Pyoungtaek area.

Background
: The purpose of this study is to define the clinical characteristics of Tsutsugamushi disease.

Methods : We analyzed 12 cases of tsutsugamushi disease admitted to BAG-AE general hospital from 1999. Oct. 1st to Dec. 10. Tsutsugamushi disease was diagnosed by indirect immunofluorescent antibody test.

Results : 1) Among them, 8 cases were female and 4 cases were male. The prevalence was predominantly high in female. 2) The age distribution ranged from 40 to 80 years old. The high incidence of age was 70's and 80's. 3) The peak incidene was showed between October and November(91.6%). 4) The most frequent symptoms were fever and chills(91.6%). 5) Rash and eschar were observed in 83.3%(10 or12) both. 6) Hepatosplenomegaly and lymphadenopathy were also observed in 25%(3 or 12), 16.6%(2 or 12) respectively. 7) The Laboratory finding showed the elevation of AST 66.6% (8 of 12), ALT 58.3% (7 of 12), alkaline phosphatse 25% (3 of 12), and LDH 25% (3 of 12). Albuminuria and hypoalbuminuria were also observed in 25% (3 of 12), 58.3% (7 of 12) respectively. 8) The therapeutic efficacy of doxycycline was excellent.

Conclusion : Tsutsugamushi disease which appeared in Pyoungtack area showed typical clinical pattern and was effective to doxycycline treatment.

The effect of aromatherapy on dysmenorrhea from high school girls.

Background
: This study is to investigate on how much the dysmenorrheal and lower abdominal pain(LAP)/back pain are improved by the aromatherapy which is emerging as an alternative therapy.

Methods : In order to get the data by questionnaire, 245 subject was selected from women student class of a high school at rural area from June 2000 to August 2000. 125 subject, aroma was given to 69 subject and placebo was given to 56 subject. The subject was investigated how the dysmenorrhea was improved by using the aroma and placebo.

Results : After the aromatherapy, severities of dysmenorrheal and LAP/back pain improved in aroma group and placebo group. Therefore, there is not statistical difference between both groupts.

Conclusion : The herbal aroma is not effective more than placebo aroma to relieve dysmenorrhea.

Association between family APGAR and depression for one university freshmen.

Background
: Though many adolescents are suffering from depression in Korea, there have been few studies about the prevalence of it and depressive adolescents are not managed appropriately yet because of ignorance of the society and family. This study was intended to find out college freshmen's prevalence of depression, and the relationship between family function and depression.

Methods : The subjects were college freshmen of one university both in Seoul and Suwon and the study period was from January to February, 1999. Family APGAR was used as a family function index Beck's Depression Inventory (BDI) was used to measure the degree of depres-sion. Chi square tests and logistic regression were done for analysis using SPSS 8.0/PC program.

Results : The prevalence of depression among college freshmen was 1.5% (37 out of 2465) as of BDI score 21 or more. Depression was more common in the families with low Family APGAR score and those in Suwon campus. No significant differences were found in gender, age, blood pressuere, blood type, BMI, present illness, number of families, marital status of parents or siblings. Relative risks of depression were 4.5 times for family dysfunction group and 2.6 times for Suwon campus group.

Conclusion : Depression is not a rare disease among college freshmen. Therefore, family physician as a primary care physician should consider the possibility of depression among adolescence and screen depression using BDI for adolescents in dysfunctional family particularly.

Diagnostic value of dipstick urinalysis as a screening test for urinary tract infection.

Background
: Urinary tract infection is one of the most common diseases in primary practice. Although quantitative urine culture is the standard for diagnosis, a rapid screening test is needed for effective management. But numerous studies in the past were controversial. We evaluated the utility of dipstick urine analysis as a screening test for urinary tract infection.

Methods : A retrospective analysis of 311 patients who were admitted between May 1997, and April 1998 were chosen as subject. Patients had a complete urine analysis, such as microscopic analysis, chemical analysis by dipstick and urine culture was performed as part of an evaluation for urinary tract infection or suspicious febrile illness.
We compared dipstick analysis with microscopic examination and urine culture. The results of the dipstick urinalysis were compared with the results of the urine culture, sensitivity, specificity, positive predictive value and negative predictive value.

Results : A total of 311 cases were included in the study among which 122 cases were true pos-itive in urine culture. For dipstick urine analysis 92 of 122 cases were leukocyte esterase positive and 39 of 122 were nitrite positive. On microscopic examination 44 of 122 cases were leukocyte positive and 40 of 122 were microorganism positive.

Conclusion : The sensitivity of the dipstick urinalysis seems higher than the microscopic exami-nation in urinary tract infection. We conclude that the dipstick urinalysis is a useful screening test for urinary tract infection for those above 2 years old.

Psychogenic symptoms in patients with noncardiac chest pain.

Background
: Patients with chest pain are common among outpatients. Several researches shows that chest pain patients can suffer from psychiatric illnesses such as hypochondriasis, depression, anxiety, somatization, panic disorder and so on. This study aims to examine the psychogenic symptoms in patients with noncardiac chest pain.

Methods : In this study, the 98 people who have undergone treadmill test for chest pain during the three months from June 1st to September 30 of 1998 were selected, and the 98 patients were asked to respond to the prepared questionnaire and SCL-MPD(symptom check list-minor psychiatric disorders) before they underwent treadmill test. And then, they were classified into two groups. One group consisted of patients with noncardiac chest pain, and the other group of patients with cardiac chest pain. These two groups were compared in 19 measures, and the compared results were analyzed by t-test.

Results : Among the 25 patients with cardiac chest pain, 15 patients were males and 10 patients (33%) were females, and among the 44 patients with noncardiac chest pain, 31 patients(70%) were males, 13 patients (30%) were females. The average age was 47 years old. The average age in patients with noncardiac chest pain was 48, and the 46 for those patients with cardiac chest pain. In the statistical analysis between the patients with cardiac chest pain and those with noncardiac chest pain, significant difference(p<0.005) was shown in the measures such as somatization, depression, phobic anxiety, obsessive-compulsive, neurasthenic, hypochodrial, anxiety, anger-hostility, interpersonal sensitivity.

Conclusion : As in the analysis the patients with noncardiac chest pain showed significant difference in the symptom check list minor psychiatric disorder compared to those with cardiac chest pain. We conclude that primary care physician should take more interest in evaluating psychiatric symptoms in patients with noncardiac chest pain.( J Korean Acad Fam Med 1999;20:1784-1793)

The significance of fundus photographs during health mass screening.

Background
: Fundus photographs performed during health check-up were reviewed to determine the usefulness in mass screening.

Methods : Subjects consisted of 3,017 people who had undergone physical health check up at the Heath Clinic, Dong-Eui Medical Center in 1997 and whose fundus photographs of both eyes were taken showing clearly visible posterior pole and optic disc were chosen as subjects. Medical records which included cases of reported close exam and causes, actual cases of close exam and final diagnoses,

Results : Of the 3,017 people, reported close exam was performed in 665(22.0%). Among them glaucoma was suspected in 460 retinal hemorrhagic lesion in 63, retinal degenerative lesion in 67 and others in 75. There were 187 actual cases(28.1%) of close exam including glaucoma suspect in 79, retinal hemorrhagic lesion 47, retinal degenerative lesion 34 and others 27. Among 187 actual cases of close exam, 140 people (1.3%) were eventually diagnosed as glaucoma suspect(60), retinal hemorrhagic lesion(38), retinal degenerative lesion(21) and others(21)

Conclusion : Fundus photographs are useful in mass screening, but performing lose exam to enhance the usefulness of the fundus photographs is necessary.(J Korean Acad Fam Med 1999;20:933-939)

Comparison of the clock drawing test with the Mini-Mental State Examination as a cognitive screening tool for dlderly patients..

Background
: Cognitive impairment often goes unrecognized by physicians because the many previous tools to evaluate cognitive function in the clinical setting are difficult and time-consuming. The purpose of this study was to compare the Clock Drawing Test(CDT) with the Mini-Mental State Examination(MMSE) as a cognitive screening tool for elderly patients.

Methods : In a hospital-based outpatient clinic, 99 patients aged 65 or older were administered with the MMSE and were asked to mark the digits in the pre-drawn clock. They are also evaluated with the Geriatric depression scale, the Activities of Daily Living, and the Instrumental Activities of Daily Living.

Results : Correlation between CDT and MMSE was high(r=-0.606; p<0.001). With respect to CDT, there were significant correlations with all subscales of MMSE except registration of memory(p<0.01).
CDT with scores of 2 had a sensitivity of 68.2% and a specificity of 70.9% for mild cognitive impairment. CDT with scores of 4 had a sensitivity of 80.8% and a specificity of 84.8% for severe cognitive impairment.

Conclusion : CDT can be an effective and simple cognitive screening tool for the elderly patients in the primary care outpatient clinic, if compensated for registration of memory among the cognitive function.

A Study of Psychogenic Symptoms in Chronic Low Back Pain Patients.

Background
: Chronic low back pain(CLBP) is one of the most common afflictions of humans. Between 70% and 80% of the population experience back pain some time during their lives. Especially, back pain is the most common cause of limited activity in individuals under age 45.

Methods : The prepared questionnaire and SCL-MPD(symptom check list-minor psychiatric disorders) were presented to subjects who visited our hospital for occupational routine check. The questionnaire was composed of duration and severity of back pain. The subjects were divided into two groups, respectively with chronic low back pain history and without history. The differences of psychogenic symptoms between both groups were analysed.

Results : In general characteristics, there was no significant difference between both groups. There were significant differences in somatization, anxiety, depression, anger-hostility, phobic anxiety, neurasthenic, hypochondriacal, interpersonal sensitivity and general dimensions between both groups(P<0.05). There was no significant difference in obsessive-compulsive dimension between both groups.

Conclusion : These results showed the presense of various psychogenic symptoms in chronic low back pain patients. Such psychosocial variables will need to receive additional careful attention in primary care setting.
