Relationship of Simple Renal Cyst to Hypertension

Background

Simple renal cyst is the most common cystic deformation found in adults. However, there were a few systematic Korean reports for the clinical symptoms and complications of simple renal cysts. The author's purpose was to determine the relationship between simple renal cysts diagnosed by abdominal ultrasonography and hypertension.

Methods

Among 13,482 persons who took a routine physical examination at Ulsan University Hospital in 2002, 5,127 persons who took medical examinations again in 2010 were selected. We excluded persons who had renal cyst, hypertension, diabetes, liver cirrhosis, kidney disease, thyroid disease, and cancer in 2002. Analysis was conducted for 505 subjects with newly diagnosed simple renal cyst and 2,744 subjects without renal cyst in 2010. The simple renal cyst group was compared to a control group without renal cyst.

Results

Among 3,249 subjects, simple renal cyst and hypertension were newly diagnosed in 505 subjects and 503 subjects. The subjects who had simple renal cysts had significantly higher hypertension incidence (odds ratio [OR], 1.53; 95% confidence interval [CI], 1.20 to 1.94). Significant hypertension incidence was observed especially when the subjects had cysts located on both kidneys (OR, 3.48; 95% CI, 2.12 to 5.71), two (OR, 3.08; 95% CI, 1.84 to 5.15) or more cysts (OR, 3.12; 95% CI, 1.38 to 7.04), and larger cysts more than 1 cm in diameter (OR, 1.55; 95% CI, 1.16 to 2.07).

Conclusion

The presence of simple renal cysts was significantly positively related to the incidence of hypertension. Bilateral distribution, two or more number, and larger size than 1 cm diameter were the characteristics of simple renal cyst related to hypertension.

Non-alcoholic Fatty Liver Disease and Chronic Kidney Disease in Koreans Aged 50 Years or Older

Background

Non-alcoholic fatty liver disease (NAFLD) and chronic kidney disease (CKD) share common pathogenic mechanisms and many risk factors, and both are linked to an increased risk of cardiovascular diseases. The aim of this study was to assess the association between NAFLD and CKD according to the presence of hypertension and diabetes mellitus in Koreans aged 50 years or older.

Methods

A cross-sectional study of 1,706 subjects who received their routine health examination was conducted between May 2008 and April 2010 at Konkuk University medical center. Biochemical tests for liver and abdominal ultrasonography were performed. CKD was defined as either proteinuria or glomerular filtration rate ≤60 mL/min per 1.73 m2.

Results

Among the 1,706 subjects, There were 545 (31.9%) with non-alcoholic fatty liver disease and 424 (24.9%) with chronic kidney disease. In univariate logistic regression analysis, NAFLD was significantly associated with CKD (odds ratio [OR], 1.69; 95% confidence interval [CI], 1.34 to 2.12). In multivariate logistic regression analysis adjusted for age, sex, current smoking, abdominal obesity, aspartate aminotransferases, alanine aminotransferases, γ-glutamyltransferase, hypertension, diabetes mellitus, hypertriglyceridemia, and low high-density lipoprotein cholesterol, NAFLD was associated with CKD (adjusted OR, 1.68; 95% CI, 1.27 to 2.24). This relationship remained significant after classification according to the presence of hypertension or diabetes mellitus.

Conclusion

NAFLD diagnosed by ultrasonography was significantly associated with CKD in Koreans aged 50 years or older.

Glomerular Filtration Rate Difference between Renal Cyst Group and Control Group.

Background
The simple renal cyst is the most prevalent cystic deformation in adults and is most of them are incidentally found during medical examination. In this study, the clinical differences were compared between simple renal cyst and control groups diagnosed by abdominal ultrasonography during periodic medical examination.Methods: We randomly selected 2,277 persons who took medical examination in one general hospital health promotion center. Among them, analysis was conducted for 188 subjects with simple renal cyst and 188 subjects without renal cyst whose sex, age, and body mass index were matched. Renal cyst subjects were compared with control group to search for their relationship with hypertension, renal function and microscopic hematuria, past medical history, social history, results of other abdominal ultrasonography findings, urine test, and blood test. Results: Among 2,213 subjects, simple renal cyst was found in 188 subjects (8.5%). The subjects who had more than three simple renal cysts were significant older (P = 0.05) and the oldest age was 70's. Also, higher hypertension prevalence (P = 0.05), more microscopic hematuria, higher serum creatinine (P = 0.02), and lower glomerular filtration rate (P < 0.01) were observed in simple renal cyst group. Conclusion: It is needed to survey size, shape and change of simple renal cyst using abdominal ultrasonography as well as how its progression may be related to developing hypertension, decreased renal function and microscopic hematuria.

Prevalence of Gallbladder Polyp in Adults in Korea.

Background
Incidentally discovered gallbladder polyp in health check-up has been increasing in numbers due to extended use of ultrasonography. Past researches have reported on the relationship between body mass index and gallbladder (GB) polyp. As there has been a recent rise in the prevalence of obesity, this research attempted to study GB polyp prevalence and the trend according to time elapse in Korea. Methods: Retrospective cross sectional research was carried out with the data of Korean Association of Health Promotion, between 2nd. Jauary. 2003 to 3rd. Aug. 2007, for subjects who received abdominal ultrasonography. Those excluded were,below 20 years of age, non-asian and who have received cholecystectomy. The number of eligible subjects were 137,135, where 69,493 were male and 67,642 were female. Results: Out of 137,135 subjects, the number of subjects with GB polyp were 5,278, where 3,163 were male and 2,115 were female. According to 2005 census, the age adjusted prevalence was 3.7% (3,656 per 100,000). The prevalence for men was 4,403 per 100,000 (4.4%) and for women 2,943 per 100,000 (2.9%). The prevalence has been increasing for the past five years. Conclusion: The prevalence of GB polyp in Korea is found to be higher than the pre-existing reports and the prevalence is higher in male than female. The prevalence is on the increase and there is a need to be more attentive to the issue in the future.

The Relationship between Body Mass Index and Mammographic Density in Korean Women.

Background: There have been very few studies on the relationship between obesity and dense breast as risk factors of breast cancer in Korean women. Therefore, we evaluated the relationship between BMI and mammographic density in Korean women. Methods: We recruited 332 women without prior history of breast cancer, breast surgery, radiotherapy, hormone replacement therapy, and family history of breast cancer who underwent screening for mammography and completed a self-administered questionnaire in one general hospital. On the basis of ACR BI-RADS breast composition, four density patterns were classified repeatedly by a radiologist. Results: The mean age, Body Mass Index (BMI), the proportion of postmenopausal women of the study population were 50.4 years, 23.2 kg/m2, and 29.8%, respectively. The proportion of extremely dense breast women was 100% in the underweight group, 77.3% in the normal weight group, 41.4% in class 1 obesity group, and 25% in class 2 obesity group, decreased with increasing BMI. The odds Ratio of BMI to extremely dense breast was 0.82 (95% CI: 0.72∼0.93, P value=0.002) after adjustment for age, menache, menopause, and BI-RADS assessment category in logistic regression. Conclusion: Our findings showed that the BMI was negatively correlated with mammographic density as well in Korea. Therefore, primary physicians should consider sensitive additional method for breast cancer screening especially in underweight and normal weight Korean women. (J Korean Acad Fam Med 2008;29:831-837)

The Association of Nonalcoholic Fatty Liver Disease with Metabolic Syndrome.

Background
Recently, the diagnosis of non-alcoholic fatty liver disease (NAFLD) has been made more frequently, as the use of ultrasonography on health screening has been on the rise. The aim of this study was to elucidate the relationship between NAFLD and the metabolic syndrome defined by NCEP-ATP III criteria. Methods: A total of 1,675 subjects, who attended for routine physical check‐ups, were screened. Among those, 401 subjects were selected after excluding the subjects with either significant alcohol consumption, evidence of viral or toxic hepatitis, significant liver or renal dysfunction, or overt thyroid disease. NAFLD was diagnosed if the subjects had a "bright liver" on ultrasonographic examination. The diagnosis of metabolic syndrome was made according to the criteria of NCEP-ATP III established in 2001. Results: The prevalence of NAFLD was 27.2%. Blood pressure, body weight, body mass index, waist circumference, and serum levels of total cholesterol, triglyceride, fasting glucose and liver enzymes were higher among the subjects with NAFLD than the control. The prevalence of obesity, IFG/DM, dyslipidemia and hypertension was higher in the NAFLD group. The prevalence of the metabolic syndrome was 19.2%, which was higher in the NAFLD group than the control group. The odds ratio of NAFLD for the metabolic syndrome was 6.458 (95% C.I. 3.178∼13.124). Conclusion: NAFLD was closely associated with the metabolic syndrome regardless of the presence of obesity. (J Korean Acad Fam Med 2007;28:667-674)

Background

Methods : Twenty young male patients with 26 shoulder pain cases were prospectively included in this study. All had a physical examination performed by Cyriax's selective tension method. As a diagnostic gold standard, ultrasonographic examination was carried out within one week of the physical examination by two specialists.

Results : The most common findings by sonographic evaluation were sub-deltoid bursitis (65.4%). Sub-deltoid bursal effusion without rotator cuff injury were 10 cases (58.8%) and 17 cases (41.2%) showed combined rotator cuff lesion. The most common diagnoses by physical examination were sub-deltoid bursitis (46.2%) followed by supraspinatus tendon lesion (19.2%). Clinical assessment by physical examination showed low sensitivity in the diagnosis of rotator cuff lesion (42.9%) and sub-deltoid bursitis (70.6%) but high sensitivity and specificity in the diagnosis of shoulder joint instability (sensitivity 100%, specificity 91.7%).

Conclusion : This study suggest that the validity of Cyriax's selective tension test for the diagnosis of shoulder pain are questionable.

Mondor's Disease of the Breast: Clinical Manifestations and Imaging Findings.

Mondor's disease or thrombophlebitis of the superficial veins of the breast is a rare benign condition and a self-limiting disease. The combination of the clinical manifestations and the imaging (mammographic & sonographic) findings is the typical sign of Mondor's disease of the breast. Therefore, an understanding of such an entity and knowledge of the clinical and imaging findings will make it possible to avoid the unnecessary biopsy or surgery. We report the case of a 53-year-old woman diagnosing as Mondor's diasease, who underwent evaluation of a tender, papable cord- like mass in her left breast associated with discoloration of the overlying skin.

Comparison of the Diagnostic Value between Mammography and Mammography with Breast Ultrasonography in Diagnosing Breast Cancer.

Background
: Because the sensitivity of mammography (MAM) is lower in young women than in old and in women with dense breast than those without, many physicians utilize breast ultrasonography (US) with MAM. But the addictive value of US is unclear. This study compared the diagnostic value between MAM and MAM+US.

Methods : Between September, 2002, and February, 2003, there were 89 cases of breast disease confirmed by surgery, that had both MAM and US results. The results of MAM and US were classifed by Breast Imaging Reporting and Data System (BI-RADS) and were categorized as positive or negative. Among the positive results, BI-RADS Category 0 and Category 3 with recommendation of other test were regarded as an intermediate result.

Results : Among all 89 cases, 38 cases were malignancies. The receiver operating characteristic curves showed non-significant increase in diagnostic value by adding US to MAM. The sensitivity of MAM was non-significantly higher in above-50-years women than in below- 49-years women and in women with dense breast than in women with non-dense breast. Among positive results of MAM, 21 cases were 'intermediate results'. US correctly regraded 8 benign lesions from 19 cases of false-positive MAM and 1 extra malignancy with palpable lump from 4 false-negative MAM.

Conclusion : MAM is a valuable test in diagnosing breast cancer. US is a valuable complementary test to MAM in evaluating palpable lump with negative MAM and intermediate result of MAM.
