Body Weight Perception and Weight Loss Practices among Private College Students in Kelantan State, Malaysia

Background
Body image is associated with the perception of people on themselves. Influencing factors are generated internally and/or externally. The most common issue pertaining to body image is body weight and weight loss. This study aimed to determine the association between body weight perception and weight loss practices among college students.
Methods
A cross-sectional study was conducted involving 297 college students from private nursing colleges in the state of Kelantan, Malaysia. A self-administered questionnaire was used to assess sociodemographic characteristics, body weight perception, and weight loss methods. Weight and height were measured, and body mass index (BMI) was calculated based on weight and height measurement. The World Health Organization BMI cutoffs were applied in the study.
Results
More than half (54.2%) of college students perceived their weight correctly as per actual measured BMI. A total of 51.5% of participants had tried various methods to reduce their weight. Body weight perception is associated with weight loss practices (odds ratio, 0.31; 95% confidence interval, 0.19–0.50; P<0.001) adjusted for sex, marital status, and status of having obese family members. Those who had correct body weight perception were less likely to engage in weight loss practice. Food intake restriction (42.4%) is the most popular weight reduction method among students in nursing colleges. Over a quarter of the participants chose physical exercise (25.3%) to reduce their weight, and a small number engaged in unhealthy weight loss practices.
Conclusion
Body weight perception is an important factor that influences the practice to reduce weight especially among young adult group and college students.

Pro Re Nata Prescription and Perception Difference between Doctors and Nurses

Background

Pro re nata (PRN) prescription is a frequently used prescription method in hospitals. This study was conducted to investigate actual condition of PRN prescription and whether administration error occurred because of perception difference between doctors and nurses.

Methods

From May to July 2012, a survey was conducted among 746 doctors and nurses (88 doctors and 658 nurses) working at 5 hospitals located in Seoul, Gyeong-gi, and Gangwon Province. Doctors generating PRN prescription responded to actual conditions of PRN prescription and both doctors and nurses reported whether administration error occurred due to perception difference.

Results

Average number of PRN prescription of surgical residents was 4.6 ± 5.4, which was larger than that of medical residents (1.7 ± 1.0). Surgical residents more frequently recorded maximum number of daily intake (P = 0.034) and, although not statistically significant, more often wrote exact single dosage (P = 0.053) and maximum dosage per day (P = 0.333) than medical residents. Doctors expected nurses to notify them before the administration of medication; however, nurses were more likely to conduct PRN administration by their own decision without informing doctors. In addition, some doctors and nurses experienced administration errors because of it.

Conclusion

Standard prescription methods need to be established since there is a perception difference in PRN prescription between doctors and nurses and this could be related to administration errors.

A Comparison of Clinical Oncologists and Family Physicians toward Complementary and Alternative Medicine in South Korea: Perceptions, Attitudes and Physician-Patient Communication.

Background
Although, its efficacy is unclear, the application of complementary and alternative medicine (CAM) is increasing among patients worldwide. The physician's role is very important to help patients understand and use CAM properly. The aim of this study is to examine the perceptions, attitudes and physician-patient communication of about CAM in two distinguished specialty physicians. Methods: A web-based survey was performed to list oncologists and family physicians in Korea between May 2007 and July 2007. E-mails were sent to total of 5,429 physicians and 547 physicians replied on survey (response rate: 10.1%). Of them, 500 participants comprising 174 clinical oncologists and 326 family physicians were involved in this study. The study questionnaire consisted of 26 questions including questions about general characteristics of CAM and intentions of introduction and education to their patients. Results: Of 500 physicians, males were 73.4%, and 82.0% were middle-aged 30-49. Family physicians had more interest than oncologists about CAM (79.5% and 57.5%, respectively). In addition to, family physicians preferred introducing CAM to their patients than oncologists (34.1% and 18.4%, respectively). Almost 85% of the oncologists and family physicians thought that it was necessary to introduce CAM to their patients. However, 60.2% of them had hesitated their practice because 70.3% of them did not have suffi cient knowledge in CAM. Both specialty physicians responded that usefulness of CAM information when regarding practice, should contain evidence-based evaluation (59.2%) and consensus guideline(37.0%). Conclusion: The results of this study showed that most physicians did not have sufficient information regarding CAM(87.2%). The result implies that evidence based information on complementaryand alternative medicine should be offered in the national level to improve communication between physicians and patients.

Factors Related to Human Papilloma Virus Infection Rate in Women.

Background
It is not clear which demographic factors would be considered for human papillomavirus (HPV) infection in Korean women and how women understand the HPV infection. This study was conducted to fi nd the factors related to HIV infection and the realities of follow-up tests and perception among women infected with HPV. Methods: The subjects were 2,113 women (median 41 years old) who received a Papanicolaou smear and a HPV DNA test at a health promotion center from March 2006 to October 2007. The follow-up rate and perception for HPV infection among women infected by HPV were surveyed using telephone. Results: Overall prevalence of HPV infection was 17.3% (N = 365). However, the prevalence of HPV infection among women younger than 30 years old reached 30.7% (17.6% for low-risk group of HPV, 13.1% for high-risk group of HPV). The odds for being infected with HPV was 3.2 (95% confi dence interval, 2.0 to 5.2) among unmarried women younger than 30 years old compared to those at the ages of 30 years or older. Married women younger than 30 years were also 2.1-4.3 times more likely to be infected with low-risk group of HPV compared to those aged 30 years or older. Among those infected with HPV and answered to the survey (N = 282), 51.4% took irregular gynecologic follow-up and 66.2% did not have information for the need or importance of follow-up examination. Conclusion: The risk of being infected with HPV seems to be higher for unmarried women younger than 30 years old. Additionally, a standardized guideline should be provided to improve follow-up examination and knowledge about HPV infection.

Family Physicians' Perception on Hearing Impaired People.

Background: Hearing impaired people have a lot of difficulty in communicating when they visit medical institutions. This study was to reveal the family physicians' perception and attitude for the hearing impaired people, and the effect of those factors on the actual physicians' practice behavior for the disabled. Methods: We sent questionnaires by post mail to 1,000 members listed in the 2007 address book of the Korean Association of Family Medicine. Results: Among the total of 90 family physicians who had answered, 72 (80.8%) had willing attitude to treat the hearing impaired people, and the most important reason was that it was essentially a doctor's duty. In the group who were unwilling to treat the disabled, the most principal reason was that they were not equipped with sufficient facility and medical system. Gender was the only factor affecting the physicians' attitude significantly and the female doctors had a higher intention to treat the hearing impaired people than male doctors (P=0.035). Age, location of hospital, number of patients, practical experience of hearing impaired people had no significant effect on doctors' attitude. The number of physicians who volunteered to participate for the hearing impaired people's health promotion personally was significantly higher in the group of treatment intention (P=0.007). Nevertheless, few physicians had the willingness to equip the hospital facility for the hearing disabled, even in the group of willingness to treat the disabled. The most important reason was that very few disabled patients visit their clinic, and so forth they felt no necessity to improve and re-organize their clinical environment. Conclusion: Most family physicians are willing to treat hearing impaired people. But even in the willing group, almost all of them are unwilling to improve or equip the medical facilities for the hearing impaired people personally, because only a few disabled people visit the primary care hospital in the real practice. Relationship between Body Mass Index and Perception of Body Image in School-aged Children.

Background
Body image, a personal perception of the body shape, is known to influence motive to control body weight. This study aimed to evaluate perception of body image according to the weight status in children. Methods: The study participants were 442 Korean boys and girls enrolled in 2nd or 6th grade of a primary school. Heights and weights were measured by standardized method. Each child was asked to fill in a questionnaire asking about the factors which might be probably related to body image and to select a figure reflecting their current and ideal images among 7 drawings grading the extent of fatness. Weight status was classified using the age- and gender-specific cut-off points of the International Obesity Task Force based on the body mass index calculated. Chi-square test and multiple linear logistic regression test were performed. Results: Among the subjects, 17% of the children were overweight, and 4.3% were obese. Among the overweight or obesity group, the children who chose figures corresponding to fatty body image were 4% and 32%, respectively, and the children who wanted to be thinner were 27% and 47%, respectively. Only the previous trial for weight reduction was independently associated with body image distortion defined as being satisfied with current body image in overweight or obese children. Conclusion: Safe and systematic efforts providing children with adequate body image perception would be needed to prevent and manage the obesity problem, especially for overweight or obese children who think their body images as normal or who were satisfied with their current body images. Lifestyle and Health State in Individuals with Frequent Nocturnal Sleep Interruption.

Background
: Many studies have demonstrated that excessively long or short sleep duration and insomnia are associated with high risk of mortality. We intended to investigate the lifestyle and the state of health in frequently sleep interrupted people.

Methods : We have examined 2,540 subjects who visited a comprehensive medical testing center from January 2001 to June 2003. We divided into two groups of sleep disturbance into frequently interrupted group (two or more times per night) and infrequently interrupted group (less than twice per night). After frequency matching by age and sex, 1,900 subjects (each group of 950 subjects) were selected. Lifestyle, self-health perception, symptoms, clinical laboratory data and past history were compared by chi-square test and student's t-test. To identify the factors that influenced on sleep interruption, multiple logistic regression analysis was done.

Results : Sleep duration was not significantly different in both groups (P=0.486). The frequently interrupted group were likely to perceive themselves as bad state of health being in a (P=0.008) and to complain of various symptoms, i.e., fatigue (P<0.001), headache (P= 0.001), etc. Also, they had more incidence of mood disorders (P= 0.002), fatty liver (P=0.008), and arthritis (P=0.001) than the other group. By multiple logistic regression analysis, alcohol consumption (OR: 1.344, P= 0.007), irregular diet (OR: 1.325, P=0.003), and coffee intake (OR: 0.786, P= 0.013) were significantly different between the frequently interrupted group and the other group.

Conclusion : Patients who complain of insomnia should be evaluated on both sleep duration and interruption. The evaluation of sleep interruption is needed in patients who perceive themselves as being in a bad state of health, who complain of various symptoms (fatigue, headache, etc.), or have a history of mood disorder, fatty liver, or arthritis.

Factors Associated with Distorted Self-perception of Body Weight in Korean Adults.

Background
: This study was done to examine the relationship between distorted self-perception of body weight and some sociodemographic factors in Korean adults.

Methods : This study was conducted among 14,621 adults (men 10,833, women 3,788), who visited Ulsan University Hospital Health Promotion Center to take a routine health examination. The objective body weight status was classified as underweight, normal, overweight, and obesity by body mass index. We compared the self- perception of body weight with in objective body weight status. We used multiple logistic regression to assess the odds ratio for distorted self-perception of body weight by age, marital status, monthly income, and level of education.

Results : Among the total 72.8% of obese men had a distorted self-perception of being slightly excess, 48% of overweight men considered themselves as just about right, and 39.5% of men with normal body weight as underweight. In women, 36.5% with normal body weight had a distorted self-perception of being slightly excess or very excess. In both men and women who were overweight or obesity, the distorted self-perception of being just about right was increased with higher age and lower education. The odds ratio of distorted self-perception in normal as being slightly excess or very excess was higher in women with higher income and higher education.

Conclusion : In both men and women, many people had a distorted self-perception of Body weight. Some sociodemographic factors were found to be related to the self- perception of body weight.

A Survey on Perception of Obesity in General Population.

Background
: In modern society, thin appearance is treated as ideal, making the majority of the normal weight group think that they are fat. Therefore, exercise and diet, numerous unhealthy methods were frequently chosen by normal weight group for weight control. The authors gave a definition of Ideal Body Weight and attempted to propose a new classification of obesity to the normal weight group who do not view themselves as they should.

Methods : The test subjects were 261 adults, who visited the IJUH Health Promotion Center between May 15, 2002 and June 30, 2002. After the subjects were measured anthropometric values such as height and weight, the self-recorded questionnaires including 12 questions were collected. The grades of somatotype drawing were 1∼9 by BMI. The subjects chose 1 somatotype drawing that was thought to be obese man and woman.

Results : The data were collected from 261 subjects. For female somatotype drawing, 81.7% of the men and 49.1% of the women thought the drawings of normal weight were obese (P<0.001). For male somatotype drawing, 47.7% of the men and 29.3% of the women thought the drawings of normal weight were obese (P<0.01). For women's weights, 81.7% of the men and 82.1% of the women thought the normal weight was obese. For men's weights, 20.3% of the men and 23.6% of the women thought the normal weight was obese. Women thought the normal weight was obese more than in men.

Conclusion : Women thought the normal weight was obese more than in men. Many men and women thought below the BMI 25 was obese.

Parental perception on childhood obesity according to weight status in children of elementary schools in Busan.

Background
: It has been found that parental role is essential for obesity management in childhood. However, there are few studies concerning parental on childhood obesity in Korea. Parental perception on childhood obesity according to weight status in elementary school children was examined.

Methods : The subjects were 3,996 elementary school children and their parents in Busan. The surveys which included socioeconomic characteristics, children's eating habits and activities, parental weight and height, parental perception on childhood obesity, were filled out by parents. Children's body mass index(BMI) were calculated using the height and weight measured in school. Children were classified into the obese group (BMI≥95 percentile), the high risk group (85 percentile≤BMI<95 percentile) and the normal weight group (<85 percentile) according to BMI gender and age. The analyses were done using x²-test and ANOVA.

Results : Parental BMI in the obese group was higher than the normal weight group in boys and girls (P<0.001). Participation in regular exercise was higher among the obese group compared to the normal weight group regardless of sex, but daily exercise time was lesser among the obese group only in boys (P<0.001). Daily TV watching time was not different according to weight status in both sex. The parents with obese children perceived their children's weight as an appearance problem or a health problem about 7-18 times more likely than the parents of children with normal weight. 70% of obese children have tried weight control. The proportion of parental understanding on childhood obesity as a disease was not different among boys, but different among girls (P<0.001). Eighty five percents of the parents perceived childhood obesity as a disease or a risk factor. But, their perception for the association of hypertension, fatty liver, sleep apnea syndrome with obesity was relatively low.

Conclusion : Most parents with obese children perceived childhood obesity as an appearance problem, a health problem and as a disease or a risk factor.

The factors relates to the screening tests of colorectal cancer in men.

Background
: Cancer is the second leading cause of death in Korea. Colorectal cancers rank fourth in term of total cancer mortality. Colorectal cancers have been the focus of screening for early detection. Few current data are available regarding factors associated with participation in cancer screening examinations in the general population. This study investigated associations of age, income, education, marital status, smoking, drinking, sleeping, presence of a hemorrhoid, and attitudes toward cancer prevention with participation in three cancer screening tests for colorectal cancer.

Methods : To identify the factors associated with participation in cancer screening examinations, 2372 men in a certain company were surveyed in 1992. Cancer related questions were included to determine the attitudes toward cancer prevention and the history of tests performed that are used for screening for colorectal cancer.

Results : Statistically significant (P<0.05) strong predictors of ever having had a specific cancer screening test were as follow:(1) digital rectal examination: above 40 years old(OR=3.4, 95% CI 2.1-5.4), having a hemorrhoid(OR=2.9, 95% CI 1.7-5.0), and regular exercise(OR=2.1, 95% CI 1.2-3.5); (2) fecal occult blood: above 40 years old(OR=4.2, 95% CI 2.5-7.2) and (3) colonoscopy or sigmoidoscopy: above 40 years old(OR=3.5, 95% CI 2.4-5.2), married(OR=3.5, 95% CI 1.6-7.7), and heavy drinker(OR=1.8, 95% CI 1.2-2.7).

Conclusion : The largest differences among the population for participation in cancer screening examinations involved age. Education and encouragement may persuade men participate more in screening.

The factors related with office worker's health perception.

Background
: Recently, the concept of health is extended to physical, mental and social aspects, which leads people to pursue health behavior positively. But the evaluation of health condition shows a tendency to be biased to functional side. It this case, even if he is in physically good health, he is not rated as healthy. We analyzed the relationship between one's health state and the factors related with health perception including health behavior, mind and social environment.

Methods : For a month of May 1996, we surveyed those who visited for periodic medical check-up at one general hospital in Seoul. The questionnaire consisted of the total of 25 items ; diet behavior, weight control, coffee, coke, tea, drug use of, smoking, drinking, exercise, leisure, safety belts use, jay walking, sleep, stress, personality, anxiety, depression, occupational satisfaction, relationships with family and friends. Five point scale is used as the answer of each question item with high point as good living behavior. Among 406 who had sincerely answered the questionnaire, we divided them into healthy and unhealthy group and compared with the point of each question item and with the laboratory data(obesity, blood pressure, hemoglobin, fasting blood sugar, cholesterol and ALT).

Results : Among 406, the number of those who perceived themselves to be healthy was 175(43.1%, healthy group) and the one of those who did not was 231(56.9%, unhealthy group). The difference between both groups was not significant in distribution of sex, age, academic career, income and religion. In laboratory data and living behavior they showed same results, also. But the points of unhealthy group were significantly lower than those of healthy group in coffee, coke, tee, drugs usage, exercise, leisure, stress, personality, anxiety, depression, occupational satisfaction and relationship with family and friends. So was the total point of all items of unhealthy group in comparison with that of health group. In the case of male, the points of unhealthy group were significantly lower in smoking, safety belt use, jay walking, sleeping as well as the above items. On the contrary, in the case of female, both groups were not significantly different all items except exercise and leisure.

Conclusion : Health perception is influenced by psychosocial factors and living behavior more than by physical health. Therefore family physician must be concerned about patient's living behavior, mental and social problems and must make an effort to change their perception of health as well as physical improvement.

A survey on change of perception and satisfaction of family medicine (From patients add in one general hospital in seoul).

BACKGROUND
The author surveyed and analyzed the changes of perception and satisfaction about family medicine. So that the outcome may bring us a reference to advance family medicine more.
METHODS
247 in and out patients in one general hospital in Seoul were surveyed by the questionnaire from July 26, 2000 to June 20. The date was analyzed statistically by SAS program.
RESULTS
As for the knowledge of family medicine, 61.5% of the patients answered that they knew what it is, and the most of them, 54%, via medical institution. The office workers, married , and highly educated people tend to know more about family medicine (P<0.05). As for attitude, 68% of the group answered that family medicine is necessary, because "family medicine doctors give proper advice and consult through out all fields of medicine"(58.8%). As for the preference so prirate doctor, 56.7% of the group answered that they would register for family medicine clinic, and students and office workers are fond of family medicine(P<0.05). As for the satisfaction, 34.4% were ever been to family medicine clinic, and 35.5% out of them were dissatisfied, mainly due to the lack of knowledge of physicians about their diseases(70.1%).
CONCLUSION
The perception of family medicine are still scanty compared to the old studies.
