Abstract
Background
: Fundus photographs performed during health check-up were reviewed to determine the usefulness in mass screening.
Methods : Subjects consisted of 3,017 people who had undergone physical health check up at the Heath Clinic, Dong-Eui Medical Center in 1997 and whose fundus photographs of both eyes were taken showing clearly visible posterior pole and optic disc were chosen as subjects. Results : Of the 3,017 people, reported close exam was performed in 665(22.0%). Among them glaucoma was suspected in 460 retinal hemorrhagic lesion in 63, retinal degenerative lesion in 67 and others in 75. There were 187 actual cases(28.1%) of close exam including glaucoma suspect in 79, retinal hemorrhagic lesion 47, retinal degenerative lesion 34 and others 27. Among 187 actual cases of close exam, 140 people (1.3%) were eventually diagnosed as glaucoma suspect(60), retinal hemorrhagic lesion(38), retinal degenerative lesion(21) and others(21)
Conclusion : Fundus photographs are useful in mass screening, but performing lose exam to enhance the usefulness of the fundus photographs is necessary.(J Korean Acad Fam Med 1999;20:933-939)
초록
연구배경 : 건강검진센터를 통한 종합검진시 시행되는 안저사진의 집단거민에서의 유용성에 대해 알아보고자 하였다.
방법 : 1997년 1년 동안 동의의료원 건강검진센터에서 종합검진을 시행한 대상 중 양안 모두의 후극부와 시신경유두가 명확히 나타나는 안저사진이 보존되어 있는 3017명(6034안)을 대상으로 하여 정밀 검사를 통보 받은 경우와 그 원인, 실제 정밀검사를 받았는지 여부, 최종적 진단에 대해 후향적 조사를 시행하였다.
결과 : 3017명 중 정밀검사를 통보 받은 경우는 녹내장이 의심되었던 경우가 460명, 망막의 출혈성 병변이 있었던 경우가 63명, 망막변성이 있었던 경우가 67명, 총 665명으로(22.%) 이중 실제 정밀검사를 받은 경우는 녹내장의심 79명, 망막출혈성 병변 47명, 망막변성 34명, 기타 27명으로 187명(28.1%)이었다. 최종적으로 이상 진단을 받은 경우는 140명으로 녹내장 60명, 출혈성 병변 38명, 망막 변성 21명, 기타 21명이었다. 녹내장의 경우 정상안압 녹내장이 46명으로 가장 많았으며, 망막출혈성 병변의 경우는 26명에서 당뇨가, 망막변성이 있었던 경우는 11명에서 퇴행성 근시가, 기타의 경우는 12명에서 고혈압이 가장 많은 빈도를 보였다.
결론 : 안저사진은 집단검진을 유용하여, 그 유용성을 더욱 높이기 위해 정밀 검사를 통보 받은 대상에서 재검사율을 높이기 위해 위한 방법이 모색되어야 한다고 생각한다.
