Abstract
Background
: In order to treat obese individuals, control of caloric intake after determination of patient's energy expenditure and recommendation of exercise program with possible use of specific medication is advised. There are many limitations in measuring all individual's energy expenditure by calorimetry, thus estimation is made using many variables such as weight, height, age, and gender. But, commonly used equations to predict resting energy expenditure(REE) are thought to result in overstimation of energy expenditure for use with obese individuals. Methods : Subjects were 133 adults who were admitted either to Internal Medicine Department and ENT Department of Yeung Nam University Hospital and their resting energy expenditure was measured indirest calorimetry between groups was compared. Predicted REE estimated by 6 equations to measured REE was compared in the two groups by body mass index 27 kg/m² under 65 years old individuals.
Results : There was no significant difference of measured REE between groups according to smoking habits(p>0.05). In the older group(≥65 years old), REE was lower compared to the younger groups(<65 years old)(p<0.05). In the obese group(body mass index>27kg/m²) measured REE was higher than the normal weight group(BMI<27kg/m²)(p<0.05). Among 108 indibiduals under 65 years old, other equation except for Fleish equation and Robertson and Ried equation were inaccurate when applied to the obese group.
Conclusion : These commonly used prediction equations tend to overestimate the REE in obese individuals, and the best estimates for the obese seem to be derived from the Fleish and Robertson and Reid equations.

연구배경 : 비만을 가진 환자들을 치료하려 할 때에는 환자의 에너지 소비량을 결정하여 에너지 섭취를 조절하고 운동요법과 약물요법 등을 고려할 수 있다. 에너지 소비를 결정하기 위해서 직접 측정하는데에는 여러가지 제약이 있으므로, 키, 신장, 연령 등을 이용한 공식들을 이용하여 계산할 수 있다. 그러나, 기존의 공식을 이용한 측정법은 과체중인 사람들에서 안정시 에너지 소비를 과다예측할 소지가 있다. 본 연구는 기존 에너지 소비 예측공식중 과체증군에서 실제 측정치와 근접한 공식을 찾고자 하였다.
방법 : 1996년 5월 11일에서 1998년 6월 30일사이에 영남대학병원 내과, 이비인후과에 내원하여 간접 칼로리 측정법을 시행한 환자 133명을 대상으로 실제 측정된 안정시 에너지 소비를 비교하였다. 65세 미만인 환자 108명을 대상으로 체질량지수 27kg/m²을 기준으로 두 군으로 나누어 6가지 예측공식들로 계산한 안정시 에너지 소비량과 실체 측정치를 비교해보았다.
결과 : 흡연습관으로 나눈 세 군간의 에너지 소비는 유의한 차이가 없었다(P>0.05). 65세를 기준으로 두 군으로 나누어 실제 측정치를 비교한 결과 65세 이상인 그룹에서 유의하게 낮았으며, 체질량지수가 27이상인 군에서 27 미만은 군보다 유의하게 안정시 에너지 소비 측정치가 높았다(P<0.05). 65세미만은 108명중에서 체질량지수가 27이상인 과체중군(n=36)에서는 Fleish 공식과 Robertson and Reid 공식만이 유의한 차이를 보이지 않았다.
결론 : 비만인 군에서는 일반적인 공식들로 계산한 예측치가 실제 측정치를 과다예측하는 경향이 많았으며, 체표면적으로 이용한 공식인 Fleish 공식과 Robertson and Reid 공식이 적합할 것으로 생각된다.
