The study on the stress amount and life event according to family life cycle.

가족생활주기별 스트레스량 측정 및 생활사건의 내용분석에 관한 고찰

Abstract
Background
: It is important to analyze the stress amount and content of life event according to family life cycle through the recent experience of life event in family practice, because it need to predict disease and understand the relationship among the family members. This study was attempted to establish data base for health promotion and disease management and assist perform the comprehensive and continuous care of the family practice.

Methods : The data were collected from married adult 486 of the above 20 age, from July to September, in 1992. Family life cycle by Duvall and S.R.R.S. by T.H. Holmes and R.H. Rahe was selected and analyzed the stress amount in terms of their recent experience of 43 kinds of life events and induced the 6 life event in the order of frequency, according to family life cycle.

Results : The result were summarized as follows
1. According to increase the family life cycle, mean stress amount was tending to decrease.
2. First 5 life events by frequency about 486 subjects were vacation(35.2%), christmas(28.2%), major change in argument with spouse(22.8%), minor violation of the law(18.7%), change in residence(17.5%).
3. Mean stress amount by abnormal life event associated with family life cycle was higher in diseased group rather than non-diseased group

Conclusion : There were some significant difference in mean stress amount and content of life event according to family life cycle. these finding have implication for the comprehensive and continuous care of the family practice.
It is important to calculate the stress amount in transitional zone. But this study was not noted the finding in transitional zone. Therefore further study in tansitional zone will be needed.
초록
연구배경 : 최근의 생활사건을 통하여 스트레스를 양적으로 분석하고 그 가족생활주기에 따라 생활사건의 내용을 분석하는 것은 가정의학과 진료에 있어서 질병의 예측 및 가족기능의 파악을 위해서 필요하다고 여겨지는 바, 가족생활주기에 따른 스트레스량과 생활사건의 내용을 빈도별로 분석하여 건강 및 질병관리를 위한 기초자료를 얻기 위해 본 조사를 시행하였다.

방법 : 1992년 7월 1일부터 1992년 9월 1일까지 2개월 동안 20세 이상의 건강한 결혼한 성인 850명을 대상으로 1967년 Holms와 Rahe가 만든 총 43문항으로 된 설문지를 배포하여 지난 1년간 일어났던 생활사건의 빈도수를 조사하였으며, 설문에 응답한 537명 중에서 분석에 부적당한 51례를 제외한 486례를 대상으로 Duvall의 가족생활주기에 따른 스트레스량을 SPSS/PC에 의하여 비교분석하였다.

결과 : 총 486명 중 남자 268명, 여자 218명으로 남여 비는 1:1.2였고 평균 연령은 41세(남자 40세, 여자 43세)였다.
조사대상 486명의 평균 스트레스량은 163이었으며 각 가족생활주기별 평균 스트레스량은 Duvall stage 1에서 246, Duvall stage 2에서 224, Duvall stage 3에서 171, Duvall stage 4에서 148, Duvall stage 5에서 140, Duvall stage 6에서 126, Duvall stage 7에서 69, Duvall stage 8에서 103으로 가족생활주기에 따라 감소하는 경향을 보였다.
가족생활주기에 따른 생활사건의 빈도별 상위사건은 Duvall Stage 1에서는 결혼(64.2%), 휴가(41.8%), Duvall stage 2에서는 자녀의 출생(54.3%), 임신(47.1%), Duvall stage 3에서는 휴가(57.1%), 크리스마스(41.1%), Duvall Stage 4에서는 휴가(41.9%), 크리스마스(32.6%), Duvall stage 5에서는 본인 또는 자녀의 입학 또는 졸업(40.3%), 휴가(29.2%), Duvall stage 6에서는 자녀가 집을 떠남(26.7%), 집안의 새식구가 생김(24.0%), Duvall Stage 7에서는 수면습관의 변화(24.1%), 가까운 친구의 죽음(20.7%), Duvall stage 8에서는 가까운 친지의 죽음(32.3%), 가까운 친구의 죽음(29.0%) 등의 순으로 나타났다.

결론 : 이상으로 가족생활주기별로 스트레스의 내용과 양에 있어서 많은 차이가 있음을 알 수 있었으며 가정의학과 환자 진료시 이에 대한 고려가 필요하리라 여겨진다.
