Motivation and Time to Apply for Family Medicine Resident.

Abstract
Background
As of 2006, the residents of family medicine occupies about 9 percent among the total. However, expansion of more general practice doctors is in demand to conduct desirable general practice. Accordingly, this study was conducted to survey the motivation to apply for family medicine residency and the actual situation. Methods: A questionnaire survey of 862 residents of nationwide family medicine from June 2006 to July 2006 was performed via e-mails. Hospital, size, years, gender, age, university, career, time and motivation to apply, and existence and nonexistence of lecture in medical school or clinical practice of family medicine for students were reviewed. Results: Among the total, 415 (48.14%) residents from 72 training hospitals answered. For the motivation, 'the necessity to diagnose and treat various patients' was 53.49%, 'advantageous to start practice' was 21.93%, and 'relatively short training period' was 11.33%. For the time, during 'intern' was 49.40%, 'on getting a job' 16.63%, and as a 'student' 15.42%. When they had lectures of family medicine in medical school, decision made as a student was 19.93%, and during clinical practice was 21.61%. Conclusion: Among the reasons that motivated to apply for family medicine, 'the necessity to diagnose and treat various patients' was the biggest, which conforms to the training goal of Academy of Family Physicians. Since there is a tendency to select family medicine residency earlier when they are exposed to lectures of family medicine or clinical practices in medical school, the effort to expand substantial lectures and clinical practices is needed. 초록
연구배경: 2006년 현재, 가정의학과 전공의는 전체 전공의의 약 9% 정도를 차지하고 있지만, 바람직한 일차진료의 수행을 위해서는 더 많은 일차진료의사의 확충이 요구된다. 이에, 가정의학과 전공의들의 전공의 프로그램 지원동기 및 실태를 알아보고자 본 연구를 시행하였다. 방법: 2006년 6월부터 7월까지 전국 가정의학과 86개 수련병원 전공의 862명을 대상으로 전자우편을 통한 설문조사를 시행하였다. 소속병원 및 규모, 연차, 성별, 나이, 출신학교, 가정의학과 전공의 시작 전 경력 및 지원결정 시기와 동기, 의과대학 재학시절 가정의학과 강의 및 실습(수련) 프로그램 유무에 대하여 조사하였다. 결과: 전체 전공의 862명 중, 72개 수련병원 415명(48.14%)이 설문에 응답하였다. 가정의학과를 선택하게 된 동기는 '다양한 환자진료의 필요성'이 53.49%로 가장 많았고, '개업에 유리할 것 같아서'가 21.93%, '수련기간이 상대적으로 짧아서'가 11.33% 순이었다. 가정의학과를 선택한 시기는 '인턴 때'가 49.40%로 가장 많았고, '취직했었을 때' 16.63%, '학생 때'가 15.42% 순이었다. 의과대학 재학 중에, 가정의학 강의가 있었던 경우에는, 학생 때 결정한 경우가 19.93%였고, 학생실습이 있었던 경우에는, 학생 때 결정한 경우가 21.61%였다. 결론: 가정의학을 전공과목으로 선택하게 되는 동기는 '다양한 환자진료에 대한 필요성'이 가장 많았고, 이는 바람직한 일차진료의사 양성이라는 학회의 수련목표와 일치한다. 학생 때 가정의학 강의나 실습이 있었던 경우에, 더 일찍 가정의학을 전공으로 선택하는 경향이 있으므로, 내실 있는 강의와 실습을 확대해 나가는 노력이 필요하겠다.
