가정의학과 외래 진료내용 분석을 통한 전공의 수련과정 평가

Abstract
In order to evaluate the resident's training in ambulatory care, 6,437 health problems seen by 15 family residents during 8 months of 2nd resident year and 4 months of 3rd resident year in family practice center were collected and analyzed by descriptive statistical method.
The general findings are as follows;
1. Residents saw an average of 326.9±129.4 patients during one year in family practice center, and spent 91.2±18.7 half days on an average, so residents saw an average of 3.6±1.3 patients each half-day.
2. The 4,904 patients seen by 15 residents was composed of 46.4% males and 53.6% of females, and it had no significant difference between each resident(P<0.05). Patient less than 15 years old were 3.4% and those older than 65 years old were 22.3%. Generally the proportion of younger age group was small but the propotion of the older was variable.
3. Residents saw an average of 429.1±185.0 health problems, and average health problems per patient visit were 1.4
4. Residents used an average of 79.2±18.7 health problems among the total of 362 from ICHPPC.
5. The cummulative percentile from the most frequent to the 10th frequent health problems accounted for 47% of total. Of 6,437 health problems seen by 15 residents, the most frequent health problem was uncomplicated hypertension and 2nd and 3rd frequent health problems were functional GI disturbance and abdominal pain respectively.
6. According to the classification of disease category, the category 16 (sign, symptoms, ill-defined conditions) which accounted for 27.9% of all health problems was most common and the categories 9(digestive system) and 7(circulatory system) were in the next place, but residents were experienced a few cases in ophalthmology, ENT, categories 2(neoplasms), 11(pregnancy, childbirth, puerperium), 14(congenital malformation) and 15(perinatal morbidity & mortality).
