Level of Understanding of Importance of Preventing and Treating Dermatophytoses in Diabetic Patients.

Abstract
Background
Dermatophytoses tend to have a poorer outcome in diabetic patients than in non-diabetic patients. Therefore, diabetic patients need to pay a closer attention to the prevention and treatment of dermatophytoses. The aim of this study was to determine the level of understanding of the dermatophytoses in diabetic patients and to clarify the factors associated with the level of understanding. Methods: A questionnaire survey was carried out from June to July, 2005, targeting 200 patients who had visited two hospitals in Seoul for the management of diabetes. There were 163 respondents (response rate 50.9%). The degree of understanding was calculated by giving 1 point to each question, with a maximum of 12 points. Results: The average degree of understanding of dermatophytoses was 6.68 points (SD 1.79). Younger age (P= 0.0002), high education level (P=0.0035), and high income (P=0.0013) were the factors related to a better understanding of dermatophytoses. The patients who had obtained the information on dermatophytoses from the websites, broadcasts, newspapers, or magazines showed higher recognition points than others (P=0.0377). The body mass index, the duration of diabetes, the complications associated with diabetes did not affect the understanding of dermatophytoses. Conclusion: There are many diabetes patients who do not know enough about dermatophytoses. Diabetes itself is not related to the level of understanding of dermatophytoses. However, diabetic patients who are older, have a lower level of education, and a low income will need to be informed about prevention and treatment of dermatophytoses. 초록
연구배경: 당뇨병 환자에서 백선 발생 시, 괴사나 궤양, 이차적 세균감염의 발생률이, 백선이 없는 당뇨병 환자에 비해 증가하게 되므로, 당뇨병 환자는 백선의 예방과 치료에 더욱 신경 써야 한다. 본 연구에서는 당뇨병 환자를 대상으로, 백선 인식도와 이에 관련된 요인을 알아보고자 하였다. 방법: 2005년 6월부터 7월까지 서울 시내 두 곳의 2차병원 내과에 입원하거나 외래에 내원한 당뇨병 환자 중, 배포된 설문지를 완성한 163명(응답률 50.9%)을 대상으로 단면적 연구를 시행하였다. 결과: 백선 인식도는 총 12점 만점에 평균 6.68점(SD 1.79)이었다. 낮은 연령(P=0.0002), 높은 학력(P=0.0035), 높은 소득(P=0.0013)이 높은 인식도와 관련 있는 인자였다. 인터넷, 방송, 신문, 잡지를 통해 백선 정보를 얻는 군은 그렇지 않은 군에 비해 인식도가 높았다(P=0.0377). 체질량지수, 당뇨병 이환기간, 당뇨병 합병증은 백선 인식도 및 백선 경험과 유의한 관계가 없었다. 결론: 당뇨병 환자들 중 백선에 대한 인식이 낮은 사람이 많다. 당뇨병 자체는 백선 인식도와 큰 관련이 없지만, 높은 연령, 낮은 학력, 낮은 소득의 환자들에게 인식도 향상을 위한 교육이 더욱 필요하다.
