| Abstract |
Background
: primary physicians should understand not only the biomedical status but also the functional capacity of their patients. There are Many instruments designed to measure functional status, but none of them are confirmed to be valid and reliable in Korean version. This study was designed to develop Korean version of COOP/WONCA charts which are approved to be reliable and valid through many international studies, and to evaluate their validity and reliability.
Methods : The charts were translated to Korean language and evaluated face validity. The patients who visited family medicine outpatient clinic of one hospital from June to July in 1994 were included in this study. They administered COOP/WONCA Charts and MOS-20 measurement which was also translated. In order to test the validity, the correlation codfficients between the same items of COOP/WONCA Charts and MOS-20 measurement were calculated. Then the convergent and discriminant validity were evaluated via Multitrait-Multimethod approach. To test the reliability. 30 persons who completed the COOP/WONCA Charts readministered the same charts after 1 hour, and 22 persons of them administered again after 1 or 2 weeks when they revisited the clinic. The test-retest correlations and Cronbach's alpha coefficient were calculated.
Results : The average convergent validity correlation between COOP/WONCA Charts and MOS-20 measurement was 0.5232, and ranged from 0.3502 to 0.8714. Most of them met the 5 criteria proposed by Campbell and Fiske. 1-hour-test-retest intraclass correlations ranged from 0/7160-0.9529, and 1 or 2-week-test-retest correlations ranged from 0.3516-0.7250. Cronbach's α coefficient was 0.7937.
Conclusion : Developed Korean version of COOP/WONCA charts are reliable, valid, and useful for quickly measuring patient function in primary care setting. But those items which showed lower convergent validity coeffcients such as physical fitness and social activities should be reevaluated. |
| 초록 |
연구배경 : 일차 진료의사들은 환자의 생의학적 상태 아니라 기능상태까지도 파악하고 있어야 한다. 기능상태를 평가하는 도구들은 이미 많이 개발되어 소개된 바 있으나, 아직 한국어판으로 타당도 및 신뢰도가 증명된 것이 없는 실정이다. 본연구는 국제적인 임상 연구를 통하여 일차진료에서 기능상태를 평가하는 표준으로 인정되고 있는 COOP/WONCA charts의 한국어판을 개발하고 그 신뢰도 및 타당도를 평가하는 것을 목적으로 하였다.
방법 : 먼저 기능상태평가 도표를 번역하여 명목타당성을 검증하였고, 1994년 6월부터 7월 사이 일개 대학병원 가정의학과 외래를 방문한 환자들을 대상으로 기능상태평가 도표를 배포하고 스스로 작성하게 하였다. 타당도의 검증을 위해서 한국어판 기능상태평가 도표와 따로 번역된 MOS-20 설문지를 사용하여 두 설문지의 각 항목간 상관계수(Spearman's correlation coefficient)를 구하였다. 이후 다특성-다방법 접근(MTMM approach)을 통하여 구성개념 타당도(Convergent and discriminant validity)를 평가하였다. 또한 신뢰도의 평가를 위하여 도표를 작성한 사람 중 30 예에서는 1시간 뒤에 다시 설문지를 작성하게 하였고, 22 예에서는 1주 혹은 2주 후 재방문시 다시 작성하게 하여 각 도표간의 상관관계를 조사하였으며, 내적 합치도를 보기 위해 Cronbach's alpha를 구하였다.
결과 : 기능상태평가 도표와 MOS-20 설문지를 모두 작성한 238예에 관하여 분석한 결과, 수렴 타당도(convergent validity)계수는 0.3502-0.8714로 평균 0.5232였고, Campbell과 Fiske가 자세한 5가지 기준을 대체로 만족시켰다. 1시간후 재검사에서 Spearman 상관계수는 0.7160~0.9529, Kappa계수는 0.4239~0.8134였으며, 1~2주후 재검사에서는 각각 0.3516~0.7250, 0.1613~0.6196이었다. 또한 α 계수는 0.7937이었다.
결론 : 개발된 한국어판 기능상태평가 도표(COOP/WONCA charts)는 일차진료시에 비교적 간단하게 환자들의 기능상태를 평가하는 방법으로서 타당하고 신뢰성이 있는 것으로 평가된다. 그러나 비교적 낮은 수렴타당도를 보인 '운동 능력', '사회적 활동' 문항에 관하여는 재검토가 필요할 것으로 사료된다. |
